Salvia should be used with a sober sitter in a quiet, safe room. Every public health source on this page gives that advice, and the shop that sells the extracts prints it on its product pages. The reason is practical: at higher amounts people lose contact with their surroundings for several minutes, may stand up and walk, and do not remember it afterwards. Most harm linked to salvia comes from that, from falls and bumps, and from using it in the wrong place (FRANK).
What the experience asks of the room
Smoked salvia acts within a minute. In laboratory studies the effects peaked at about two minutes and the strongest part was over within roughly twenty (Johnson 2011). At high amounts participants often reported the maximum intensity the questionnaire allowed, and many could not respond to the people around them for a while (MacLean 2013). Chewed leaves start more slowly, after five to twenty minutes, and last up to two hours (ADF).
That timing shapes the setup. There is no time to tidy up once it starts.
- Sit or lie on a sofa, bed or mattress on the floor, away from stairs, balconies, open windows and sharp corners.
- Clear the floor. Move glasses, candles and anything breakable out of reach.
- Lighters and pipes go back on a table after use, out of the user's hand.
- Dim light, no loud music, phones on silent.
- No pets in the room.
Do it indoors, in a private space. A park, a beach or a shared hostel room is the wrong place.
What the sitter does
The sitter stays sober for the whole session and does not use salvia at the same time. One sitter per user is the simplest rule.
Before. Agree on a few things while everyone is clear-headed: how much will be used, that the sitter may take the pipe away, and a simple phrase the sitter will repeat if needed, such as "you are in the living room, you are safe, it will pass in a few minutes".
During. Stay close and quiet. Take the pipe and lighter as soon as the smoke is inhaled. Do not hold or restrain someone unless they are about to get hurt; guide them gently back to the seat instead. Keep talking to a minimum: people under the influence often find voices confusing. Note the time.
After. Most people come back within 15 to 30 minutes after smoking. Some want to talk, some want silence. Give them water and let them sit. Nobody leaves the house, cycles or drives until they are fully clear. In the Netherlands driving any vehicle under the influence, a bicycle included, is an offence (Road Traffic Act, art. 8).
When to call for help
Call 112 if someone has difficulty breathing, is injured, has a seizure or does not come round after the expected time. In the Netherlands you can also call the Drugs Infolijn of the Trimbos Institute on 0900 1995 for advice.
Tell the emergency services what was used. They are there to help, and the information changes what they do.
Who should not use salvia
- Anyone under 18. Dutch shops, including Avalon Magic Plants, sell to adults only.
- People who have had a psychosis, or who have close relatives who have. Salvia can trigger psychotic episodes in people who are vulnerable to them (FRANK; RIVM 2022).
- Anyone with a heart condition or high blood pressure. The Dutch RIVM lists a raised heart rate and blood pressure among the effects seen even at the amounts recommended on packaging.
- Anyone who is pregnant or breastfeeding.
- Anyone who has been drinking or has taken other substances. Combinations with alcohol cause nausea (ADF), and in a ten-year review of Californian poison centre calls, cases involving other substances made up a large share of those with complications (Vohra 2011).
Afterwards
In a survey of 500 users, most described no lasting effects, while 4.4 percent reported lasting negative effects, mostly anxiety (Baggott 2010). If something from a session keeps bothering you, talk about it with someone you trust or with a drug information service.
